We require at least yearly updates to your case history to keep current with your healthcare needs.

We require advance notice if you need to change an appointment. This allows the opportunity for others to be treated, or you may be charged a fee.

We caution that not keeping your appointments may cause a delay in correction and lengthen your recovery.

Our Financial Policies

Insurance

If you have chiropractic coverage through your insurance company, we will bill them for you. We ask that you pay in full at the time of service. Arrangements can be made depending on your deductible and co-pay amounts. If you have benefits, your insurance will reimburse you or we will refund you when we are paid by the insurance company.

Time of Service Discount - Out of Network Plans - Large Deductibles

If we are not billing insurance for you, we do offer a discount on the chiropractic services if you pay at the time of service and your account balance is zero.

Medicare & Medicare Replacements

Medicare will pay a portion of the chiropractic adjustment after you have met your annual deductible. We are non-participating providers in the Medicare program. We require payment at the time of service and Medicare will reimburse you. In most cases, Medicare will forward the claim to your secondary insurance for reimbursement. If they do not, please contact our office for assistance.

Worker's Compensation

You have a right to choose chiropractic care. In order for your services to be covered, you must report your injury to your employer. Your employer must then file a claim with their worker's compensation insurer. We will call your employer to get the name, address, phone number and claim number of the insurance we are to bill. You may be responsible for any amount that is not covered.

Auto Accident

You may have coverage for your care under your auto policy if you are injured in an automobile accident. If another party is at fault, your services may be covered by their policy. If your claim is denied or you have reached maximum benefits, we will then bill your health insurance. You will be responsible for any amount not covered.

This is for general information purposes - please contact the office with any specific concerns.
